Sobre nosotros
As an approved provider of services for Florida’s Agency for People with Disability (APD), Proxicares’ partnership alignment with APD has collectively served that mission to specifically address and serve the needs of Floridians in Miami with developmental disabilities.
What disabilities does APD serve? APD serves more than 50,000 Floridians with developmental disabilities as defined in Florida Statutes, Chapter 393. This includes individuals with:
- Autism
- Cerebral palsy
- Spina bifida
- Intellectual disabilities
- Down syndrome
- Prader-Willi syndrome
- Phelan Mcdermid syndrome
- Children age 3-5 who are at a high risk of a developmental disability
Proxicare provides resources within our services to support people who have developmental disabilities and their families while participating, and learning in their communities.
Proxicare currently offers skilled nursing, private and residential nursing and respite skilled nursing. In addition, we also provide Personal Support, Life Skills Development Companion, and Respite Care. Additional services such as Behavior and Mental Health will be added in the next coming year of 2024. We work in accordance to the rules and regulations set by Medicaid and APD to ensure that all services are provided in compliance with the rules while adding value to the lives of each person in need. We strive to provide welcoming and safe services for the disabled community.

PROXICARE, INC. is committed to providing quality health care and work in excellence to help persons with disabilities to improve activities of daily living and emotional comfort, to help them achieve their desired goals, outcomes, and maintain and improve the service delivery system for the Home and Community Service Waiver.
We are approved by the Agency for Persons with Disabilities to provide the following services:
Registered Nurse/License Practical Nurse:
Skilled Nursing
Private/Residential Nursing
Respite Skilled Services
Home Health Aide/Certified Nursing Assistance:
Assistance with Personal Support of activities of daily living.
Life Skill Services to support interpersonal skills and psychosocial competencies.
Respite Care: Care while the family caregiver is unable to perform the duties. Usually due to a brief planned emergency absence.

Nursing Progress Note
We process our progress notes in Kinnser. The attached nursing progress note is to be used by RN's and LPN's that need a form to document during the visit however the note will need to be finalized in Kinnser. in addition, these progress notes are available in cases where the EVV (electronica Visit Verification) system is down or in emergency cases where you do not have access to EVV system. Iconnect clock in and out EVV system is required by APD Medicaid. Training will be provided within the first 7-14 days of beginning services.
